Publications
These toolkits include practical tools that will support
continued planning and implementation in the SITTAP sites. These
resources are also designed to educate and inform other communities
and the field about how they can more effectively pursue
community-based systems reform. The current toolkits address topics
such as:
- Building Community Partnerships;
- Building Sustainability;
- Family-Centered, Culturally Competent Partnerships; and
- Using Data Effectively (Recently Published).
Copies of the toolkits are $10 each with an additional $2 for
shipping and handling for the first copy. Additional copies have a
shipping and handling cost of $1. A set of 3 toolkits is available
for purchase at a total cost of $25 (not including $4 shipping). The
entire set of toolkits is available for purchase at a total cost of
$32 (not including $5 shipping). All orders must be received in
writing by fax, e-mail, or mail.

Building Effective Community Partnerships (350 KB PDF)
This toolkit
is designed to provide ideas and linkages to other resources that
will increase the capacity of demonstration projects engaged in
systemic reform efforts to bring together organizations and
individuals, develop shared goals, and implement strategies to
achieve them. It offers case study examples and a variety of tools
that communities may want to use as they consider plans for
implementing, monitoring, and institutionalizing these strategies
based upon the opportunities and needs of that particular community.

Building Sustainability in Demonstration Projects for Children,
Youth, and Families (500 KB PDF)
This toolkit is designed to
provide ideas and linkages to other resources that will enable
demonstration sites to build on their success and sustain effective
efforts beyond the life of the grant. It offers case study examples
and a variety of tools communities may want to use as they consider
plans for sustaining, replicating, or "scaling-up" reform efforts.

Family-Centered, Culturally Competent Partnerships in Demonstration

This
toolkit is designed to provide ideas and linkages to other resources
that will increase the capacity of demonstration projects engaged in
systemic reform efforts to partner with communities and families in
the development of family-centered, culturally competent approaches.
It offers case study examples and a variety of tools communities may
want to use as they consider plans for implementing, monitoring, and
institutionalizing family partnership and culturally competent
policies and practices.

Using Data Effectively: A Toolkit for Practical

This toolkit is designed to provide ideas
and linkages to other resources that will increase the capacity of
demonstration projects engaged in systemic reform efforts to use
data in a manner that allows them to accurately assess their needs,
design and implement appropriate interventions, and monitor their
progress and outcomes. It offers case study examples and a variety
of tools communities may want to use as part of their strategic
planning process and ultimately, to improve outcomes by enhancing
the quality of critical decisions about programs and services.
Toolkit #5: Systems Reform: A Primer for Creating and Sustaining Systems of Care That Work for Children, Youth and Families (620 KB PDF)
This toolkit is designed to provide ideas and linkages to other resources that will enable SITTAP demonstration sites to build on their success and sustain effective efforts beyond the life of the grant. It is a practical guide, not a cookbook, which can assist local communities in the difficult process of creating a more responsive system of care for children put at risk. This guide explores four key elements to improving systems: 1) Understanding why communities are improving systems; 2) Understanding how communities are improving systems; 3) Overcoming resistance to change; 4) Moving forward and maintaining momentum. In addition, there is a Resource List that includes numerous sources for more specific guidance on collaboration, service integration, and key elements of effective community-based systems reform.
Toolkit #6: Engaging and Partnering with Faith-Based Organizations in Initiatives for Children, Youth and Families (310 KB PDF)
This toolkit provides checklists, suggestions, case studies, and resources for how to recruit and support partners from the faith-based community in efforts to strengthen, improve, or redesign systems of care for children, youth, and families. The information is intended to help SITTAP demonstration sites connect to faith-based organizations and networks that share the common purpose of helping children and their families in need of support, yet operate within a framework different from that of government and public systems.
